NHS Friends and Family Test

We would like your feedback about the care and the treatment we provide each time when you visit us or have contact with practice. It does not take long.  Its about having your say in improving our services. 

Please complete the short survey when visiting practice next time.  Survey cards are available at the waiting room or ask at the reception.  This is anonymous.

Patient Participation Group (PPG)

The practice has a PPG if you would like to have your say in improving the servies we provide, then please Join our PPG. For further information ask at the receiption.  Everyone is welcome to join and have their say. We organise  meetings 3 times a year at the surgery or by telephone.

Comments & Sugessetions

We are always keen to improve our services and welcome your feed back, so if you have any helpful suggestions please let us know. Also, we would welcome your feedback on the service that we provide,  a suggestion box is located in the entrance area of the waiting area. 

Summary Care Record

There is a Central NHS Computer System called the Summary Care Record (SCR). It is an electronic record which contains summary medical  information.  For example, medical conditions, medicines you take,details of allergy to medicines.

Why do I need a Summary Care Record?

Storing information in one place makes it easier for healthcare staff to treat you in an emergency, or when your GP practice is closed.

This information could make a difference to how a doctor decides to care for you, for example which medicines they choose to prescribe for you.

Who can see it?

Only healthcare staff involved in your care can see your Summary Care Record, however, you will be asked for your persmission before they access.

If you do not want Your record to be viewed by this way then please let us know we can prevent this from happing.
