Zero Tolerance:
The practice operates a zero tolerance approach to violent and abusive behaviours (verbal or physical) towards any member of our staff.  Patient displaying these types of behaviours will be removed from the practice list.
 
Chaperone:
Chaperones are available on request. Please enquire at reception when booking your appointment, or with the doctor during your consultation. For the full chaperone policy, please ask reception.

Keep Your Contact up to date:

It is important that we have your correct contact details. If you change your telepnone number and/or address please let us know as soon as possible so we can update your record.

Sickness Certificates

You do not require a doctor's sickness certificate for any illness lasting seven days or less. Your employer may however require you to complete a self-certification form (SC2) which is available from your employer or on the www.Gov.uk website.

If you are sick for more than seven days, then please make an appointment to see GP to get a Fitnote.

To receive an update on the progress of your benefit payment please add your mobile number and/or email id to your fit note before posting. 

Digital/paperfree Fitnote option is available. Please discuss with reception or Clinician.

NHS Number

This is a unique 10 digit number allocated by the NHS at the time of birth or when you register with a GP practice for the first time. You may be asked to provide this whenever you access any NHS organisation's services to identify you correctly. 
 
If you do not know your NHS number then please ask at reception and keep it safe.
